Registering at The Practice

HOW TO REGISTER AT THE PRACTICE

To register with the practice, simply ask at reception. The receptionist will ask for your address to check that you are within the practice area and you will be given a registration form. To complete the registration process we will require photographic proof of your identity (e.g. passport/driving licence) and also proof of your address (e.g. utility bill). These documents are to be handed in with the registration form. At this point a New Patient Health Check appointment will be arranged for you with one of our Health Care Assistants.
